Notice of Availability – Form 1095-C
In compliance with the Affordable Care Act (ACA), Pepsi Bottling Ventures LLC is required to furnish Form 1095-C, which outlines the health insurance coverage we offered to you for the previous year.
Under the Paperwork Burden Reduction Act, the 2025 Form 1095-C is now available upon request. The Forms 1095 provide information about offers of coverage made to full-time employees as well as coverage information for those who enrolled in Company’s group health plan.
If you would like to receive a copy of your Form 1095-C, you can request the form through one of the following methods:

Requests can be made at any time, and the form will be provided to you by March 2nd, or within 30 days of your request, whichever is later.

If you are an active employee with access to Workday, you may log in to download your Form 1095-C at any time without submitting a request (most recent year made available by March 2nd). Log in to www.pbvbenefits.com with your Username and Password.
If you are no longer an active employee but continue to have access to Workday, you may log in to download your Form 1095-C at any time without submitting a request (most recent year made available by March 2nd). Log in to www.pbvbenefits.com with your Username and Password.
If you no longer have access to Workday, please request a copy using one of the methods provided above: email, phone, or mailing address.
